Una de las grandes mentes del rock tuvo que ceder la grandeza de la canción a un Johnny Cash de más de 70 años, quien hizo su versión la canción poco antes de morir. Pero sin duda la letra de esta canción, deprimente e incluso suicida según algunas personas, tiene algo que ver con la adicción a la cocaína que sufrió Reznor.

Puesto #1 Hurt habla simultáneamente de dos temas escabrosos. Por un lado habla de recuperarse de la drogadicción y por otro, de sobrevivir a un intento de suicidio. En cualquier caso, el protagonista de la canción se refleja en su propia experiencia en ambas instancias y se plantea un nuevo futuro en base a lo que ha aprendido.
